The online process is as follows:

● Download the PDF permission forms (see Sidebar)

● Sign them (parents/guardians also!)

● Scan them into PDFs or 1MB jpgs with your smart phone and Upload them (or mail/fax them) to us. Upload Link

You must have permission forms to us by May 24, 2024. Bring your copy just to be sure!
