Upload Your Permission Forms

In Addition to Registration, You Must Return The Following Two Forms to Attend

Complete the online Registration.

Download the forms from the download page and then

  1. Attendee must sign each form
  2. Parent or guardian signature on each form
  3. Digitize with your phone or scanner and save them as PDF
  4. Upload them from here. Stage them and finally click "Submit Form"

Each PDF file should be less than 1.5MB

Some of you may wish to use the pool or rock climbing wall. These waiver forms provide the permission to do so.
